Atlantic Beach Rescue Squad Application for Junior Membership (July 2016) Junior Membership Application

1. An Applicant for Junior Membership must be at least fourteen (14) years of age and no older than eighteen (18) years of age.
2. If Applicant is under the age of eighteen (18), a Parent/Legal Guardian signature must be submitted with the Application.
3. All information must be current and accurate; any information withheld will result in rejection of the Application. It is the responsibility of the Applicant to update any information after submittal.
4. The Application should be written or clearly printed in ink.
5. Any question that is not applicable should be marked with “N/ A”.
6. Any application not filled out correctly will be rejected.
7. The Atlantic Beach Rescue reserves the right to reject any prospective Applicant in its sole and absolute discretion.
8. Completed Applications are to be submitted to the Chief’s Office or emailed to jbkohan@abrescue.org.
9. Applicants will be notified in writing within ten (10) business days from submission of a completed application. Kindly include your email address with the application.
